This is a patient in her mid 40s that has a history of old textured saline implants (500cc size) that were placed on top of the muscle and now are severely malpositioned and contracted. When she presented to my clinic, the implants were quite hardened and very high on the chest wall. She had what is medically referred to as a “Snoopy deformity” which means the implant is malpositioned much higher than it should be, and the native breast tissue is droopy and “falling off of” the old implant. She underwent a removal of old textured saline implants, inferior pole capsulectomy, and a subpectoral “pocket conversion” to under the muscle position, as well as a replacement of silicone gel implants (450 cc moderate plus profile), and the use of Strattice acellular dermal matrix (ADM) to facilitate and support the pocket position of the implants. In addition, she underwent a breast lift in the same surgery to improve the position of the nipple and tighten and shape the breast. Her after photos are shown 5 months later. She has a very soft and natural feel to the breasts, no contracture, and an improve shape and better position of her implants.
