This is a 58 year old female with a history of having silicone gel breast implants back more than 25 years ago placed above the muscle (subglandular). She presented to my office with unhappy appearance of her breast, hardened breast implants and malposition of the implants (notice the implants appear “high” and the breast tissue is “falling” off of the implants). She underwent a removal of old ruptured silicone gel implants with a full capsulectomy from the old subglandular pocket. She had a pocket conversion to the submuscular pocket (so the new implants placed under the muscle) and a replacement of new cohesive silicone gel Allergan breast implants with ADM (acellular dermal matrix) / Strattice to prevent recurrent capsular contraction. 3 month after photos are shown with improvement to her appearance and a new soft breast without contracture and normal relationship of the breast tissue to the implant.
